How OPAT works
OPAT delivers outpatient intravenous therapy through a structured clinical process designed to maintain continuity, safety, and oversight outside the hospital setting. Each step is governed by clinical protocols and coordinated with the treating care team.
Contact your prescriber
OPAT begins with an order from a licensed prescriber who determines that intravenous therapy is clinically indicated and appropriate for outpatient delivery. Orders include therapy type, duration, monitoring requirements, and escalation parameters.
OPAT does not replace the prescriber–patient relationship. Care is delivered in coordination with the ordering clinician and documented throughout the episode.
Confirm therapy and eligibility
Once an order is received, OPAT confirms clinical eligibility, care setting appropriateness, and operational readiness. This includes review of diagnosis, access requirements, monitoring needs, and patient-specific considerations.
This step ensures that therapy is delivered within defined clinical criteria and aligns with outpatient safety standards.
Initiate care and coordination
OPAT coordinates nursing services, pharmacy fulfillment, and scheduling. Registered nurses deliver therapy according to protocol and document each visit, assessment, and monitoring event.
Care is organized as a defined episode with clear expectations, visit cadence, and communication pathways.
Ongoing monitoring and completion
Throughout the episode, OPAT provides monitoring, follow-up, and coordination with the prescribing team. Escalation pathways guide next steps if clinical indicators change.
Episodes are completed when therapy goals are met or transitioned appropriately if higher levels of care are required. Documentation supports continuity and clinical accountability.
