About OPAT
OPAT provides outpatient parenteral therapy delivered outside the hospital through structured clinical coordination and nursing oversight. The service is designed for patients who require intravenous treatment and can safely receive care in home or community settings under a prescriber-directed plan.
OPAT operates as a clinical service line, organized around defined episodes of care, documented workflows, and clear escalation standards. The focus is on continuity, safety, and completion of therapy.
Why OPAT Exists
Health systems continue to manage increasing pressure on emergency departments, inpatient capacity, and post-acute coordination. Many patients require intravenous therapy or short-term monitoring without needing prolonged hospitalization.
OPAT exists to address this gap by extending hospital-grade intravenous therapy into outpatient settings while maintaining clinical oversight and accountability. The model reflects established OPAT and care-at-home practices implemented within academic and integrated delivery systems over the past decade.
How OPAT works
OPAT works in coordination with prescribing clinicians, health systems, and care teams. Services are delivered according to written treatment plans and governed by eligibility criteria, monitoring requirements, and escalation pathways.
Care is organized as defined episodes that include:
Confirmation of clinical eligibility
Coordination of nursing and pharmacy services
Ongoing monitoring and documentation
Communication with the prescribing clinician
Defined completion criteria
This structure supports consistency and transparency across the course of care.
Clinical Oversight and Governance
OPAT services are delivered by licensed clinicians operating within applicable state and professional standards. Clinical protocols align with recognized OPAT practices and are reviewed regularly to ensure appropriateness and safety.
Escalation pathways guide transitions to higher levels of care when clinical indicators change. OPAT does not replace emergency services or ongoing medical care.
Our Approach to Patients
Patients receiving care through OPAT remain under the direction of their prescribing clinician. OPAT emphasizes clear communication, respect for patient circumstances, and coordination across settings.
Education, scheduling, and follow-up are structured to support completion of therapy and continuity of care.
