Service-line configuration defines documentation requirements at the episode level. All OPAT activity is documented using standardized workflows that support clinical oversight, compliance review, and operational analysis.
Each episode record includes:
- Eligibility determination
- Prescriber orders
- Nursing visits and assessments
- Monitoring activity
- Communication with the care team
- Episode completion or escalation
Records are structured for review by clinical leadership, compliance teams, and external stakeholders when required.
Clinical Example
A patient completes a six-week course of IV antibiotics at home. The episode record includes eligibility confirmation, weekly nursing assessments, reviewed laboratory results, documentation of therapy completion, and a closure note shared with the prescribing clinician. The record supports clinical review and external audit without reconstruction.
